Physicals and Participation Requirements

For participating in School-Sponsored/Supported Sports and High School Marching Band

Brandon Valley School District encourages co-curricular activity participation - and students in grades 7-12 have many opportunities to participate in extra-curricular activities, including marching band and school-sponsored/supported sports.

  • Students who wish to participate in extra/co-curricular activities during the 2024-2025 school year may participate, when the conditions outlined below are met. These conditions must be met PRIOR to participation.
    • A sports physical performed and signed by a doctor on or after April 1, 2024, must be submitted with the use of Brandon Valley’s 2024-2025 Physical Form - paper copies of which are available in the high school and middle school offices and in the Brandon clinics. (The Brandon Valley physical from has comprehensive sign-offs specific to Brandon Valley, allowing you to avoid having to complete/sign multiple different forms when another physical form is used.)
    • The 2024-2025 Brandon Valley physical form is signed by the PARENT and the STUDENT, and BOTH PAGES of the physical form are submitted to the Brandon Valley High School or Middle School offices.
    • The student and parent have reviewed and agree to the information on the following documents:
    • The parent has verified that all emergency information, contact information, and permissions in Family Access are updated and complete -- as this information is required and provided to coaches/advisors for their use in case of emergency/medical treatment. (Completion of the Family Access verification process is required annually.)
    • The student and parent are aware of and agree to SDHSAA Rules and Regulations found on the on the following page: https://www.sdhsaa.com/athletic-handbook/
    • The student and parent are aware of and agree to Brandon Valley School District activity rules, as outlined here: BVSD Extracurricular Guidelines.
  • By signing page 1 of Brandon Valley’s Physical Form, the parent and student agree to all of the above and are also aware of the information below.
    • Warning and Safety: Although participation in supervised interscholastic activities may be one of the least hazardous any student will engage in, by its nature, participation in these activities includes a risk of injury which may range in severity from minor to catastrophic injuries, including permanent paralysis or death. Serious injuries are not common in supervised school activity programs; however, it is possible only to minimize, not eliminate, this risk.
    • Medical Insurance: It is the responsibility of the parent/guardian to provide adequate insurance to cover any medical expenses that may be incurred while a student is participating in a school-sponsored activity. Student accident insurance can be obtained at: http://www.sas-mn.com/k12.html
